In active sentences like in the example you provided, the subject (The chef) does the action (prepared) to the object (the meal). Conversely, in passive sentences, the subject receives the action. Therefore, the example in the passive voice would look like, 'The meal was prepared by the chef'.
Something to keep in mind with the passive voice, is that the Past Participle form of the verb is always used. For example: The active voice 'She ate the cake' will change to the passive voice 'The cake was eaten by her'.
